Your pet care history shows evidence of annual wellness exams, booster exams, and care as needed.
Proof of current rabies and distemper vaccines for all pet cats and dogs, indoor or outdoor, is required.
Must show proof that dogs are on heartworm prevention if that prevention is not purchased from a vet.
All information in the following application references the adopter, who is also considered the primary caretaker of the adopted animal.
